LastPass has a deduplicator found in Settings that will consolidate your duplicate entries and give you the option to delete them. The deduplicator compares the domain, username, and password.
Go to your LastPass Vault > Account Settings > Show Advanced Settings > Click the ‘Remove Duplicates’ button at the very bottom.
